GLOBAL PERSPECTIVE

Immunosuppression practices vary widely from region to region and country to country. The one consistent tenet is that as risk for rejection increases, the level of baseline immunosuppression used increases. However, different immunosuppressive medications may be utilized to achieve this ultimate goal. Examples of this include the use of steroid-minimization or -avoidance protocols, choice of antiproliferative agent, or choice of calcineurin inhibitor.

CASE SUMMARY

• Initial clinical presentation is consistent with acute cellular rejection in a kidney transplant recipient due to subtherapeutic doses of immunosuppressive medications.

• There is a risk of opportunistic infection in patients with immunosuppression. Therefore, appropriate prophylaxis and surveillance should be employed to improve patient outcomes.
